Policies
Patients and families are informed upon admission of their rights and the procedure to file a concern or complaint. Concerns are responded to as soon as possible for any emergent concerns and resolved through the Customer Service Recovery Process within five business days or sooner if possible.
All concerns are documented on a Concern Recovery Form and forwarded to the appropriate staff member who will promptly share them with relevant supervisors and or the Chief Executive Officer and ensure tracking is completed for Performance Improvement. Performance Improvement data will be shared with the Board of Directors during quarterly meetings.
The Customer Service Recovery Process is how Hospice Care Plus responds to concerns and uses them as opportunities to examine our process, policy, practice, and staff education needs.

HCP is committed to operating in a fiscally responsible manner. HCP will not tolerate any incidents of fraud, waste, or abuse in conducting its business or delivering services to patients and their families. All federal and state laws and regulations will be followed. All employees and volunteers are responsible for acting in a way that upholds the law and adheres to high ethical standards and are expected to report any activity they become aware of that violates law or regulation.
Anyone can report unlawful activity to the Corporate Compliance Officer at HCP without fear of retribution. All reports will be kept confidential. The CEO is HCP’s Corporate Compliance Officer.

The Center for Medicare Services (CMS) requires providers to publish their rates. As a non-profit hospice, our mission is to provide care and core services to all, regardless of insurance status or ability to pay. These rates are not invoiced to patients and families; these are the rates of reimbursement CMS will pay to our organization when a patient’s care is covered by Medicare.
These rates only apply to our Home Hospice Care and Compassionate Care Center programs.
Routine Home Care (1-60 Days): $197.22 per day
Routine Home Care (61+ Days): $155.34 per day
Respite Care: $460.29 per day
General Inpatient Care: $1032.73 per day
Continuous Care: $1393.65 per day
Hospice Care Plus (HCP) provides care and services without discrimination as to race, color, creed, sex, handicap, sexual orientation, veteran status, diagnosis, or national origin.
HCP makes arrangements for care of patients/caregivers with special needs. Special needs are any psychosocial, emotional, spiritual, or cultural needs that require special arrangements. Special needs include but are not limited to physical impairment such as deafness and blindness or cultural diversity needs related to language, religion, and inability to read. Any special needs will be incorporated into the plan of care. A resource list is maintained in the Resources for Disability and Diversity policy for staff reference when a plan of care involves special needs.
Federal and state anti-discrimination and fair employment laws, as well as the federal and Kentucky Civil Rights Acts, prohibit employment practices that discriminate on the basis of age, race, color, sex, national origin, religion, disability, smoking, ethnicity, ethnic origin, veteran status, and sexual orientation. HCP will abide by these rules and regulations with regard to all HCP employees.
Any employee who believes he or she has been unlawfully discriminated against, including sexual harassment, by a coworker, supervisor, manager, physician, or agent of HCP, should report the facts of the incident and the names of the persons involved as soon as possible (preferably within 24 hours) to his/her supervisor or the CEO. Supervisors will promptly report such incidents to the CEO. All claims of unlawful discrimination will be investigated promptly, and appropriate corrective action will be taken. Corrective action may include disciplinary action up to and including discharge.
