Who qualifies for hospice care?

Any person living with an advanced serious illness such as cancer, heart disease, lung disease, Alzheimer’s, or dementia, may be eligible to receive hospice care and services.

How do I pay for hospice?

Hospice care is covered by Medicare, Medicaid, and most private insurance companies. Hospice Care Plus provides care and services to all patients, regardless of ability to pay. In nearly all cases, we do NOT invoice patients and families for co-pays, deductibles, or other expenses related to care.

How do I receive hospice care?

All you have to do is call us and ask for care. We’ll plan a visit and help determine if your loved one can benefit from and is medically eligible for hospice care. If so, we’ll work with your loved one’s physician to start care.  If not, we are happy to help you find other resources to help until it’s time for hospice care.

How does hospice serve patients and families?

Hospice care is a family-centered approach. Your hospice team works collaboratively, focusing on the patient’s and family’s needs, whether physical, emotional or spiritual. Especially at first, many families find the most reassuring services to be the nurse’s weekly visit (more, if needed), 24/7 access to nursing care, and the visits by nursing assistants. However, a social worker, chaplain, and trained volunteers are also available to assist with benefits, emotional and spiritual support, companionship, and caregiver support.
