Hospice care is designed for individuals living with a serious or advanced illness who want to focus on comfort, quality of life, and making the most of each day.
Many families tell us they wish they had learned about hospice sooner. Hospice is not about giving up. It is about receiving specialized support that helps patients remain comfortable, spend more time at home, and focus on what matters most.
When a cure is no longer possible, or when treatments are no longer providing the hoped-for benefit, hospice can help patients and families navigate the journey with expert medical care, emotional support, and practical guidance.
Home Hospice Care may be right for individuals who:
- Are living with a serious or advanced illness
- Want to focus on comfort and quality of life
- Prefer to receive care wherever they call home
- Have experienced frequent hospitalizations, emergency room visits, or increasing symptoms
- Need help managing pain or other symptoms
- Want support for family members and caregivers
- Have found that treatments are no longer helping achieve their goals of care
- Want to spend more time focusing on living and less time navigating the healthcare system
Families who choose hospice care have told us they most appreciate:
- On-call nursing support available day or night
- Nurse aids who help with light housekeeping and personal care
- Weekly nursing visits to manage pain & symptoms (and more often, as needed)
- Majority of supplies, medical equipment, and medications covered by the Medicare hospice benefit and delivered to the home
- Availability of a social worker to help with benefits and resources, and a chaplain for spiritual support
Will I get to keep my own doctors?
Yes. Your doctors are your choice. Our care team works closely with your doctor, based on your preferences. We can also provide a physician to be involved in your care, if you choose.
Do I have to wait on a doctor to refer me to hospice?
No. You, or anyone, can call Compassionate Care Plus anytime to ask about services and get general information about how we can help.
Aren't your services just for the last few days of life?
No. In fact, a national study found that, on average, patients who chose hospice care lived, on average, 29 days longer than patients who did not have hospice care. In addition, the patients reported better quality of care.
What if, right now, I just want to ask questions?
Asking questions is a good place to begin.
Whether you're exploring options for yourself, a loved one, or simply trying to understand what hospice care involves, we're here to help. Give us a call, and we'll take the time to learn about your situation, answer your questions, and help you understand the support and resources available to you.
There is no obligation and no pressure! Just a conversation.
How We Help
Whether you are navigating a serious or advanced illness or caring for someone who is, you have a lot on your mind.
We're happy to answer questions about our services, eligibility, referrals, and available resources. Whether you're seeking care for yourself, a loved one, or simply looking for information, our team is here to help. You don't have to sign up for anything to start a conversation.
When should I contact Compassionate Care Plus?
Many families tell us they wish they had reached out sooner. If you or a loved one is facing a serious or advanced illness, experiencing changes in health, or simply needs additional support, now is a good time to start the conversation.
The earlier we become involved, the more opportunities we have to provide education, resources, and support for both patients and families.
What services are right for me?
Every situation is unique. The services that may be appropriate depend on a variety of factors, including a person's diagnosis, goals of care, support system, and current needs.
Our team will take time to understand your situation, answer your questions, and help determine what services and resources may be the best fit.
How do I get started? We recommend contacting us to learn more about programs and options. You can do that here, by calling us at 859-986-1500, or via email to info@ccpky.org. Once you’re ready, we can work with your loved one’s providers to review medical records and arrange for a home or facility visit from one of our nurses. The nurse will assess your loved one and answer your questions.
